Get directionsLocated within our Health and Wellness Center, the new and modern Family Health Center provides primary medical care services in a comfortable and welcoming atmosphere to residents from the community. The Family Health Center is operated by a team of healthcare professionals that includes a physician, nurse practitioner, behavioral health therapist/social worker, diabetes educators, registered dietitians and a referral coordinator.
We offer a collaborative, team approach to caring for patients and work together to combine services in one visit, reducing the number of visits needed for care. This patient-centered practice ensures patients and members receive comprehensive care, specialty care and access to supporting wellness services.
